Guidelines for Parentage Testing Clinic Visit
1. All individuals undergoing testing should make an appointment.
1.Register under the Hematology and Oncology Department (272 clinic).
2.Schedule: Monday, Tuesday, and Thursday during morning outpatient hours (8:30 AM - 11:30 AM).
2. Required Documents:
1.Adults and legal guardians must bring "two forms of identification" (one of which must be an ID card).
2.Children:
(1) Minors with a registered name must bring their "National Health Insurance card" and a "recent household registration transcript" (the transcript should have detailed information).
(2) Minors without a registered name must bring their "birth certificate" (if the mother is a foreign national, please ensure that her identification number matches the documents).
(3) Foreign nationals must bring their "entry permit + travel permit" or "passport," as well as proof of being the "legal guardian."
(4) If the individual is already an adult, they should bring "two forms of identification."
3.Personal Genetic Sample Retention: Bring "two forms of identification."
3.Fees:
1.Parent/Child testing: Testing fee of 10,000 NT dollars per person, plus a registration and consultation fee of 492 NT dollars.
2.Grandparent/Grandchild, Sibling, or Other relationships: Testing fee of 16,500 NT dollars per person, plus a registration and consultation fee of 492 NT dollars.
3.Personal Genetic Sample Retention: Testing fee of 10,000 NT dollars, plus a registration and consultation fee of 492 NT dollars.
4.Photos: Photos will be taken on the day of the clinic visit.
5.Report Collection [Excluding holidays]:
1.Reports can be collected at the 272 clinic after 10 working days.
2.Urgent cases: Reports can be collected at the clinic after 5 working days (additional fee of 3,500 NT dollars per person).
