Department Introduction

"Child Development Collaborative Assessment Center" is equipped with a case manager who serves as a single point of contact for children in need of collaborative assessment. The center conducts joint appointments for Pediatric Psychiatry, Pediatric Neurology, and Rehabilitation Medicine every Thursday morning, providing comprehensive developmental assessments for children under six years old.

Appointment Method

Important: All appointments are now exclusively made online.

Appointment opening date: July 25, 8:00 AM.

Starting from 8:00 AM on the 25th of the previous month (if it falls on a holiday, it will be postponed to the next working day), you can make online reservations for the slots available for the next month. Once the case manager confirms the reservation, basic information and self-filled forms for parents will be sent via email. After parents complete and return the forms, the registration process will be completed.

Collaborative Assessment Process

Step 2: Come for the appointment on the scheduled date for the joint clinic of Pediatric Psychiatry, Pediatric Neurology, and Rehabilitation Medicine.

Step 3: Parents arrange assessment times with therapists as scheduled with the case manager.

Step 4: Attend the assessment at the appointed time.

Step 5: Wait for the case manager to contact you for the follow-up visit and report review.

Step 6: Arrange treatment courses.

Due to limited child assessment resources in Hsinchu County, our center strives to assist every child in receiving proper and comprehensive assessment. If you are unable to make an appointment at a suitable time, we recommend contacting other medical institutions with child development collaborative assessment centers. We apologize for any inconvenience caused.
