Department Introduction
 [Parent-Child Genetic Testing Clinic]

I. [Patients] All need to make appointments:

Register for Hematology-Oncology Department (272 Clinic).
Time: Monday, Tuesday, Thursday, morning clinic hours (8:30 AM to 11:30 AM).
II. Identification:

Both adults and legal guardians must bring [two forms of identification] (at least one must be an ID card).

Children:

(1) Minors with a registered name must bring [National Health Insurance card + recent household registration transcript] (the transcript should have detailed notes).
(2) Minors without a registered name must bring [birth certificate] (if born to a foreign mother, ensure the mother's identification number matches the documents).
(3) Foreigners must bring [entry and exit permit + travel permit], [passport], and proof of being a "legal guardian."
(4) If already an adult, bring [two forms of identification].

III. Fees:

Parent/Child: NT$10,000 per person for genetic testing, plus registration and consultation fees (NT$492).
Grandparent/Grandchild, Siblings, or Others: NT$16,500 per person for genetic testing, plus registration and consultation fees (NT$492).
Personal genetic preservation: NT$10,000 for genetic testing, plus registration and consultation fees (NT$492).
IV. Photos: Photos taken on the consultation day will be used.

V. Report Collection [Excluding weekends]:

Available for pick-up at the 272 Clinic 10 working days later.
Urgent cases: Available for pick-up at the clinic 5 working days later (additional fee of NT$3,500 per person).
