With the progress of time, there has been a proactive effort to establish the professional image of pharmacists within medical teams, making the practice of pharmacy more demanding. In addition to drug dispensing, prescription verification, and inventory management, pharmacists actively provide the public with correct medication concepts. This includes patient medication consultations, inpatient visits, public medication guidance, total parenteral nutrition (TPN) and chemotherapy drug preparation.
The pharmacy department has fully implemented computerized pharmaceutical operations, establishing a comprehensive clinical pharmacy service within the hospital. This includes medication concentration monitoring, drug utilization assessment, adverse drug reaction reporting, prescription medication evaluation, and more. "Patient medication safety" is the responsibility and mission of every pharmacist. Through continuous monitoring via the hospital information system and the assistance of barcode information operations, medications can be tracked, and appropriate integration with the nursing medication system is ensured to provide high-quality pharmaceutical services and the best oversight for public medication safety.
Inpatient clinical pharmacy services include patient medication assessment, recommendations, bedside medication guidance for discharged patients, medication concentration monitoring services, rational use management of controlled substances, participation in physician ward rounds, interdisciplinary case discussions, and clinical teaching activities.
The intelligent pharmacy warehouse management system enhances the efficiency of medication management, optimizing the workflow of medication storage operations and significantly reducing paper usage for environmental sustainability. Traditional Chinese medicine (TCM) pharmacy operations involve dispensing prescriptions from TCM clinics, inpatient prescriptions, and customized medication. The establishment of an intelligent Chinese medicine cabinet, guided by electronic labels, provides a safe and efficient dispensing environment for Chinese herbal medicines. The integration of inventory management for Chinese herbal materials in the intelligent medicine cabinet is a pioneering initiative in Taiwan and has been recognized with the 2022 SNQ National Quality Label.
With "patient medication safety" as the starting point, pharmacists combine modern information technology with education and training to safeguard the medication safety of each patient and significantly enhance service quality. Continuous efforts are made in clinical pharmacy services, focusing on cultivating excellent pharmacy talent, improving pharmaceutical service quality, and advancing clinical research and development.
